  1. Please complete New Accounts Application and Tax Form (you can type into the form). Incomplete applications will be delayed.
  2. Please print, sign and return the New Account Application form by:
  3. Retail stores and on-line retailers inside California will need a reseller number as well as a California Resale Certificate (available on website).
  4. Retailers and on-line retailers outside California need to provide a Federal Tax ID and a State Sales/Use Tax Permit/Certificate or a City/Town/County Business License.
  5. Practitioners/Physicians/Clinics need to provide a valid Practitioner or Physician license.
  6. All Retail stores, On-line Retailers, Practitioners, Hospitals/Clinics must fill out a W-9 and return it with application.
  7. Approved accounts will be set up once the first order is placed.
