- Retail stores and on-line retailers inside California will need a reseller number as well as a California Resale Certificate (available on website).
- Retailers and on-line retailers outside California need to provide a Federal Tax ID and a State Sales/Use Tax Permit/Certificate or a City/Town/County Business License.
- Practitioners/Physicians/Clinics need to provide a valid Practitioner or Physician license.
- All Retail stores, On-line Retailers, Practitioners, Hospitals/Clinics must fill out a W-9 and return it with application.
- Approved accounts will be set up once the first order is placed.
